USCIS Authorized Immigration Medical Physicals

  • When you apply for a green card (adjustment of status) in the United States, you usually need to have a medical examination which must be performed by a doctor who is authorized by U.S. Citizenship and Immigration Services (USCIS). USCIS designates certain doctors (also known as civil surgeons) to perform the medical exam required for most Green Card applicants.
  • Dr. Faiq Hasan is a designated civil surgeon authorized by the USCIS to perform Immigration Medical Examination.
  • Our designated civil surgeons will perform the required immigration medical examination according to the medical guidelines established by the Department of Homeland Security, USCIS, and Centers for Disease Control. 
  • A Complete Immigration Physical for applicants 15 years and older includes Physical examination with completion of the paper work, Tuberculin test (skin test for TB screening) and interpretation, RPR (syphilis screening), Gonorrhea screening (urine test) and Vaccine status verification.
  • Certain applicants only require a supplemental form to update their vaccination status.
  • Applicants age 2 and older are required to have a tuberculin skin test (TST) performed. This test requires a return visit to the office in a 48–72 hours time window after the TST administration. If TST reading is positive, a chest x-ray for confirmation is required.
  • Applicants age 15 and older are required to have a blood test for Syphilis screening (RPR) and a urine test for gonorrhea screening. Applicants under age 15 may require syphilis and gonorrhea testing at the discretion of the civil surgeon, or if there is reason to suspect the possibility of infection. A separate confirmation test is required if the initial screening test is positive. 
  • All candidates require proof of immunity to certain communicable diseases. This can be provided by a vaccination record or by blood test for certain vaccines. Please bring all your vaccination records with you. If you do not have any records, we can administer certain vaccines and/or blood test to determine if you have immunity. Vaccine requirements differ by age.​​

Fees

1. Complete Physical for Adults (includes Physical, skin test, blood test, urine test, vaccine status verification) $250.00
​    (Certain applicants may need additional testing if above mentioned screening tests are positive.)
2. STARTING October 1, 2018 USCIS is changing the requirements for TB screening - Skin test will no longer be used, instead TB         screening will be performed through a blood test which will cost more.

3. Supplemental form only (vaccine status verification) $75.00
